B30: What sort of information will I find in my ancestor's application
for a social security number?
The Social Security Application is referred to as: Form SS-5. The information
provided by the applicant on the form used in 1936-37 is as follows (the
form may have changed in later years):
- 1) first name- middle name- last name
- 2) street address
- 3) post office (city)-state
- 4) present employer
- 5) address of present employer
- 6) age at last birthday
- 7) month-day-year of birth
- 8) place of birth
- 9) father's full name
- 10) mother's full name
- 11) sex
- 12) color (white, negro, other-specify)
- 13) if registered with the U. S. Employment Service give number of
registration card
- 14) if previously filled out an application like this, state the place
and date
- 15) date of application
- 16) applicant's signature as usually written.
